There are two agencies who provide case management services with this funding:
Kern County Public Health Services Department’s, Health Education and Linkage Program (HELP) provides case management to individuals who are HIV-positive or have AIDS. Each client will receive an individualized care plan and a case manager to provide support, health education, and linkage to medical and social services. The goal of this program is to increase clients’ access to proper treatment of HIV/AIDS and related complications. Learn more here.
Clinica Sierra Vista (CSV) Life Linkage Program provides case management to HIV/AIDS clients to assess client’s need and develop a care plan, link clients to HIV/AIDS primary care services, ADAP, Mental Health, Dental and Vision Services. Staff work directly with client of the program and clinic providing information services, supports the program in the area, transportation, distribution of food and other basic needs.
