  1. Make an appointment through My Turn. If you need assistance, call (661) 321-3000.
  2. Complete this Back to School Registration Form prior to your appointment.
  3. Arrive on time for your appointment with your child's immunization records.
  4. If the parent/guardian of the child is not present, the parent must complete this Parent Permission Slip.

This is an appointment-based clinic. Walk-ins will be subject to the availability of staff and not guaranteed to be seen on the same day.

Yes, we are able to offer TB Risk Assessments, PPD skin tests, or TB Quantiferon testing.

Yes, however, we encourage you to utilize the Digital Vaccine Record functionality provided by the California Department of Public Health. This will provide the most current and accurate vaccine record that KCPH has access to. If you are having trouble accessing your vaccine record, contact your primary care physician’s office to ensure contact information is correct. If you are unable to access/obtain records, KCPH can provide records, and there may be a fee for providing record. 

The parent/guardian must be present with the child, unless the “parent permission slip” is completed and photo ID is provided. 

Yes, both registration on My Turn and the paper form must be completed.

Yes, insurance information will be collected, and if eligible, reimbursement will be submitted to your insurance.

Yes, there are federal programs available such as Vaccines for Children, that provide free vaccine costs to children eligible. Please note, you may still be responsible for the administration fee adopted by KCPH.

To be eligible for VFC, you must be Medi-Cal eligible/enrolled, American or Alaskan native, be uninsured, or be underinsured.
